An Honest Conversation About Cost

Why Facial Plastic Surgery Pricing Is Not a Simple Number

You will notice that most reputable facial plastic surgery practices — including this one — do not publish a price list for surgical procedures. This is not evasiveness. It is honesty.

The cost of a facelift for one patient is not the same as the cost for another — because the procedures are not the same. A deep plane facelift combined with neck lift, fat grafting, and blepharoplasty involves a different surgical plan, different OR time, and a different level of complexity than a mini facelift. A rhinoplasty that requires significant cartilage grafting and structural reconstruction is a different procedure than a focused tip refinement. Quoting a single number for either would be misleading.

What we can tell you is what goes into the cost — so that you arrive at your consultation prepared, rather than surprised. Every component of the fee is explained clearly before you make any decision.

What the surgical fee includes

Dr. Tan’s surgeon fee — her time, expertise, and the surgical plan developed specifically for you. All pre-operative appointments with Dr. Tan personally. All post-operative visits with Dr. Tan personally.

Separate from the surgical fee

Anesthesia fee — charged by the anesthesiologist independently. Surgical facility fee — the operating room, nursing staff, and equipment. Implants or specialty items if applicable.

How the total is determined

After your consultation, Dr. Tan develops your surgical plan. A complete, itemized fee estimate is provided in writing — covering every component — before you make any commitment.

Understanding Surgical Pricing

What Determines the Cost of Facial Plastic Surgery in Honolulu

These are the factors that influence the total cost of your surgical procedure. Each is explained at your consultation — before any commitment is made.

FactorHow It Affects Cost
The procedure itself
The specific surgical plan — deep plane facelift vs mini facelift, open vs closed rhinoplasty, upper vs upper and lower blepharoplasty — directly determines OR time, complexity, and surgeon fee.
Combination procedures
Most commonly combined: Facelift + Neck Lift · Facelift + Fat Grafting · Blepharoplasty + Brow Lift
Combining procedures in one surgical session (facelift + neck lift, facelift + fat grafting + blepharoplasty) adds cost to the surgical episode — but is typically more economical than separate surgeries, and requires only one recovery.
Surgical complexity and anatomy
Every face is different. Prior surgery, anatomical variation, and the degree of structural change required affect the surgical plan — and the time it takes to execute it well.
Anesthesia
Administered by an independent board-certified anesthesiologist. Charged separately from the surgical fee based on the duration and type of anesthesia required.
Surgical facility
The operating room, sterile environment, nursing staff, and post-operative recovery care. Charged by the facility independently from the surgeon’s fee.
Geographic context
Honolulu’s cost of living, facility fees, and the limited availability of fellowship-trained facial plastic surgeons in Hawaii are reflected in local surgical pricing — consistent with comparable boutique practices in major mainland markets.

Surgical Procedures

Surgical Procedure Pricing — What to Expect at Consultation

Surgical pricing is provided as a complete written estimate following your consultation — after Dr. Tan has examined your anatomy and developed your specific surgical plan. The figures below represent general context for each procedure category — not fixed quotes.

All surgical pricing includes Dr. Tan’s surgeon fee and all pre- and post-operative visits. Anesthesia and facility fees are quoted separately and included in your written estimate.

Facelift & Neck Procedures

ProcedureKey Pricing Factors
Deep Plane FaceliftTechnique complexity, degree of structural descent, OR time. Most comprehensive facelift — priced accordingly. Frequently combined with neck lift.
SMAS Facelift
Technique and extent of SMAS work. Less complex than full deep plane — appropriate for earlier-stage aging.
Mini Facelift
Shorter procedure, less OR time. Appropriate for patients in their 40s–50s with early laxity.
Neck Lift (Cervicoplasty)
Degree of platysmal work, skin excision, and whether subplatysmal structures are addressed. Most commonly combined with facelift.
Neck Liposuction
Simpler and lower cost than full neck lift. Appropriate for younger patients with isolated fat and good skin elasticity.

Rhinoplasty & Facial Procedures

ProcedureKey Pricing Factors
Upper Blepharoplasty
Upper lids only. May qualify for partial insurance coverage if visual obstruction is documented — reducing out-of-pocket cost.
Lower BlepharoplastyApproach (transconjunctival vs subciliary), fat repositioning or removal.
Upper and Lower BlepharoplastyCombined — most common combination. Single OR episode reduces incremental cost versus separate procedures.
Asian BlepharoplastyTechnique (full incision, partial, suture method). Crease height, design, and epicanthoplasty if included.
Ptosis RepairDistinct from blepharoplasty — levator advancement or tightening. May qualify for insurance coverage when functional impairment is documented.

Eyelid Procedures

ProcedureKey Pricing Factors
Rhinoplasty — Primary
Open vs closed approach, degree of structural change, cartilage grafting requirements. One of the most technically complex procedures in facial plastic surgery.
Rhinoplasty — Revision
Typically higher than primary rhinoplasty due to scar tissue, distorted anatomy, and increased complexity.
Rhinoplasty with Septoplasty
Functional component may qualify for partial insurance coverage when documented breathing impairment is present.
Facial Fat Grafting
Volume of fat harvested and recipient sites treated. Frequently combined with facelift at reduced incremental cost.
Lip Lift
In-office or OR procedure depending on extent. Shorter surgical time — priced accordingly.
Brow Lift
Technique (endoscopic vs limited incision), degree of brow descent. Frequently combined with blepharoplasty.

A note on insurance: Cosmetic surgical procedures are not covered by insurance. However, procedures with a documented functional component — functional rhinoplasty/septoplasty, upper blepharoplasty for visual obstruction, ptosis repair — may qualify for partial coverage. Dr. Tan’s team will advise on documentation requirements and pre-authorization at your consultation.

Frequently Asked Questions

Procedure & Pricing — Frequently Asked Questions

Common questions about cost, financing, and what to expect at your consultation.

Because surgical pricing depends on your specific anatomy and surgical plan — and quoting a number before examining a patient would either be meaninglessly broad or potentially misleading. What we can tell you is exactly what goes into the cost — and we provide a complete written estimate after your consultation, before you make any commitment.

Most cosmetic procedures are not covered by insurance. However, procedures with a documented functional component may qualify for partial coverage — including functional rhinoplasty/septoplasty (for breathing impairment), upper blepharoplasty (for visual field obstruction), and ptosis repair (for levator muscle dysfunction). Dr. Tan’s team will advise on documentation requirements and pre-authorization at your consultation.

Hawaii Facial Plastic Surgery accepts CareCredit — healthcare financing with interest-free promotional periods and extended payment plans. Internal payment plan options are also available for surgical procedures. Details are discussed at your consultation after your written estimate has been provided.

Contact our office for current consultation fee information. Surgical consultation fees are typically applied toward your surgical deposit when you proceed with surgery.

Yes — combining procedures in a single surgical session is typically more economical than scheduling separate surgeries. You pay one anesthesia fee and one facility fee, which reduces the total cost compared to staging the same procedures separately. The most common combinations — facelift with neck lift, facelift with fat grafting, blepharoplasty with brow lift — are discussed at your consultation.

Dr. Tan’s surgeon fee covers her fee for the surgical procedure, all pre-operative appointments with Dr. Tan personally, and all post-operative visits with Dr. Tan personally for the standard post-operative period. Anesthesia and facility fees are separate and itemized in your written estimate.

Schedule a consultation with Dr. Tan. She will examine your anatomy, develop your surgical plan, and provide a complete written fee estimate before you leave — covering every component of the cost. There is no obligation to proceed, and there is never pressure to decide at the time of your visit.
